Printed Doctor's Certificate, dated Feb 21/1866, certifying that Theodore Johnson was unable to follow his employment. Signed by H. L. Atkinson. Printed by Bro. Clarke, Printer, Moor Street, FitzRoy. Glued to the Certificate is a note to the Court from Theodore Johnson, dated Feby 21st 1866 declaring himself on the Sick Allowance of the Court. The third piece of paper, dated March 5th 1866, declaring himself off the Sick Fund of the Court as he was able to follow his employment.
