It was used in the 1950s by the Hampden Football League. This League, now known as the Hampden Football-Netball League, is in south western Victoria and was founded in 1930. This form is of minor interest as an example of the forms used by footballers wishing to transfer to other clubs in the 1950s. This is a blue sheet of paper –the form used for football players being cleared to other clubs in the Hampden Football League (4 copies). The form is blue with black print.
