Has form number CR3. Date of printing given in bottom left hand corner as 12.43. Issued to "Denmead Leslie James 133 Glenlyon Rd. Bwick" (Brunswick) in Sept. 1945. Issued at No. 1 P.D. Depot, Ransford. Has two punch holes along the top edge - probably not in at the time of issue. Has signature of holder and witness (E.A Showers) to signature of rear.
