Ethyl chloride was first used as a general anaesthetic in 1847, by Johann Ferdinand Heyfelder (1798-1869), a German surgeon. Once ethyl chloride became readily available, it was again taken up as a general anaesthetic in the late 1890s (USA). Ethyl chloride evaporates very quickly so that when it is sprayed onto the skin it produces very cold temperatures. “Refrigeration anesthesia”, or cryoanesthesia, refers to the anesthesia produced when the skin is significantly cooled.Due to its rapid onset, ethyl chloride was often used to induce general anaesthesia. It would be followed by a second anaesthetic, such as ether or nitrous oxide, which would be used for the remainder of the procedure. At the time of issue, anaesthesia was a practice still in its infancy. Proficiency in anaesthesia was determined by successful demonstration of a small number of administrations. It was also built into the existing medical qualifications and not a specialty in its own right. Dr Montefiore David Silberberg completed his training at the Melbourne and Children's Hospitals and went to London where he was clinical assistant to Sir James Mackenzie and was associated with Sir Thomas Lewis who did ground-breaking work on digitalis and electrocardiography. When he returned to Melbourne, he brought with him the first electrocardiograph used in medical practice in Australia. He started working in Collins Street and at the Melbourne Hospital as a specialist in heart problems. During World War I, he served in Egypt in 14 Australian General Hospital 1914-16. In 1919 he was appointed physician to outpatients at the Alfred Hospital while also working at the Children's Hospital and at Repatriation General Hospital. He was appointed inpatient physician at the Alfred Hospital in 1925 and, at about the same time, a University of Melbourne Stewart Lecturer in Medicine. Nitrous oxide has been used for anaesthesia in dentistry since December 1844, where Horace Wells made the first 12–15 dental operations with the gas in Hartford. Its debut as a generally accepted method, however, came in 1863, when Gardner Quincy Colton introduced it more broadly at all the Colton Dental Association clinics, that he founded in New Haven and New York City. Hospitals administer nitrous oxide as one of the anaesthetic drugs delivered by anaesthetic machines. Nitrous oxide is a weak general anaesthetic, and so is generally not used alone in general anaesthesia. General anaesthesia relaxes muscles around the airway which can result in the airway becoming obstructed. Holding the jaw open or keeping the tongue out of the way were crucial. Charles Gabriel Pravaz (1791-1853) was a French orthopedic surgeon and inventor of the hypodermic syringe. In 1844, Irish physician Francis Rynd (1811-1861) invented the hollow needle. In 1853, French physician Charles Pravaz developed the first practical metal syringe. Pravaz added a fine, hollow needle to the end of his syringe instead of the tube. This was an important innovation. Yet in the pre-antiseptic era it was a mixed blessing. The use of injections rather than oral drug administration can more readily promote the spread of disease as well as facilitating its cure. An understanding of the germ theory of disease - and the cardinal importance of using sterile needles - awaited the discoveries of Lister, Pasteur and Koch. Propofol is an induction agent for anaesthesia and also used in intensive care to induce unconsciousness. Propofol doesn't dissolve in water so it comes in a white, oily solution and must be refrigerated before it’s used. The solution consists of soybean oil, fats purified from egg yolks, and glycerol. Propofol is used as an “induction agent”—the drug that causes loss of consciousness— for general anaesthesia in major surgery. In lower doses it is also used for “conscious sedation” of patients getting procedures on an outpatient basis at ambulatory surgery centres. The main reason propofol is the agent of choice is because it allows for very rapid recovery, is the perfect drug for insertion of laryngeal masks and is the agent of choice for infusions. It has largely replaced thiopentone, the original drug of choice, but this is still available in Australia and used in specific situations. Propofol was linked to the death of Michael Jackson. The evolution of endotracheal intubation for anaesthesia was a slow process. Franz Kuhn (1806-1929), a head and neck surgeon working in Kassel, Germany, between 1902 and 1911, is said to have done more than any other to obtain general recognition for endotracheal anaesthesia. Kuhn's tube, is a flexible metal one inserted on a stilette which was then withdrawn. The tube was then secured with an elastic neck strap and connected by a rubber tube to a Trendelenburg cone. The glottis was packed with oiled gauze. Metal spiral tube with elaborate handle at one end and rounded at the other. There is a metal clasp at the base of the handle which all forms part of the endotracheal tube. Dr Bernard Johnson was elected on December the 6th December 1952 as the Faculty's first honorary Fellow. At the time, Johnson was the Dean of the Faculty of Anaesthetists, Royal College of Surgeons, England. He was invited to visit Australia to receive his award and in 1953 Johnson received his award and brought with him a notable gift being the local Dean's medal of office which was presented on behalf of the Royal College of Surgeons. Johnson had previously met with Ivan Jose and H.J. Daly by providing advice for the establishment of an Australasian Faculty. Johnson is a significant figure as a pioneer into intravenous anaesthesia into Great Britain. He then turned his attention to improving dental anaesthesia. Working with fellow colleagues R R Macintosh and W S McConnell, they established the ‘Mayfair Gas Company’, primarily for dental anaesthesia, but also provided anaesthesia for general surgery during honorary hospital sessions. Johnson participated in the development of the Faculty of Anaesthetists within the Royal College of Surgeons. A founding member of the Board of Faculty, he served as Vice-Dean and then was the second Dean (1952-5), a role which provided a seat on the council of the RCS England. A 9 carat gold [375] oval shaped Dean's Medal of the Faculty of Anaesthetists, Royal Australasian College of Surgeons secured onto a red coloured woven tape by two large gold links. Obverse depicts in coloured enamel secured onto base the RACS coat of arms and crest. Engraved around the sunken diameter are the Faculty and College names. Chloroethane, commonly known by its old name ethyl chloride, is a chemical compound with chemical formula CH3CH2Cl, once widely used in producing tetraethyllead, a gasoline additive. It is a colorless, flammable gas or refrigerated liquid with a faintly sweet odor. It is used to keep the patient's mouth open, and could also be used for oral surgery or airway management. Dr Rose was a member of the Surrey Hills Medical Centre for approximately 50 years. Norman was the son of Maurice Rose (1889-1963) and Gertrude nee Gordon (1891-1972) and was born in Birkenhead, Cheshire, UK, on 15 September 1916. Norman had an older sister, Irene. As with many migrant families, Maurice departed from London for Melbourne on 27 October 1927 on the ‘Ballarat’ ahead of his family, who sailed on the ‘Bendigo’ on 8 May, 1928. Norman was 12 years old. The family lived in a number of locations including St Kilda, Balaclava and later Elwood. Norman attended Wesley College, following which he enrolled at the University of Melbourne, where he studied medicine, graduating MB BS in 1939. For unknown reasons, post-graduation Norman moved to Perth in March 1940. It was at the Fremantle General Hospital that he met his future wife, Helen Beatrice (Betty) Mackie, and began training in anaesthetics. When World War 2 was declared, Norman enlisted in the army. He was attached to the 2/13th Field Ambulance (AIF). He served in the Middle East and Borneo. Upon their return to Melbourne after the end of the war, Norman completed hospital rotations as a Resident Medical Officer at the Alfred Hospital (1946-1947) and at the Royal Women’s Hospital in 1948. Norman was a close friend of Dr Bill Vorrath, which probably explains how he came to join the Surrey Hills Medical Centre in either 1948 or 1949. Although technically a General Practitioner, Dr Rose focused on Anaesthetics. He was often on call for the Emergency Department at Box Hill Hospital and he also taught many students how to administer an epidural. In 1972, under the so-called ‘Grandfather Clause’, he was registered as a Specialist Practitioner in Anaesthesia. Dr Rose’s long service to Box Hill Hospital was recognised with the award of Life Governorship of the Hospital in 1978. Dr Rose married for a second time after his first wife Betty died in 1997. He retired in 1998 or 1999 and died in 2010 in his 94th year. During surgery under general anaesthetic, these forceps were used to pull the tongue forward to keep the patient's airways clear from obstruction. This tool was in general use from the 1930s onwards in teaching hospitals, and became a standard piece of equipment on all anaesthetic trolleys. In the majority of teaching hospitals, the blades of these forceps were smooth and wide to cause less trauma to the tongue. Dr. Gwen Wilson, Emeritus historian, gifted this book at ANZCA's first independent Annual Scientific Meeting held in Launceston in 1994. Dr Wilson presented this gift after her speech about the life of William Russ Pugh and his significant contribution to anaesthesia in Australia. It is unclear as to how the book came into Dr Wilson's possession. Published in London, 1845, for Longman, Brown, Green and Longmans, this book was formally owned by general practitioner Dr William Russ Pugh, being the first medical practitioner in Australia to administer ether anaesthetic on 7 June 1847, in Launceston. The author of the book, P E De Strzelecki acknowledges Pugh for his assistance during his stay in Launceston and for allowing him the use of his laboratory for the analysis of the soils and minerals that were subsequently reported in this book (Page 131). Tan coloured cloth book with an embossed circular motif on the centre of the front and back cover. The same embossed motif is repeated four times along the spine of the book. An embossed design of small, four petal flowers borders the edge of the front and back cover. The title of the book 'Strzelecki's New South Wales and Van Diemen's Land' is printed in gold on the spine. The cloth has come unstuck at various places around the spine and a small section is missing from the base of the spine. The cover is worn and has numerous dark stains possibly from mould. The book contains 19 engraved and lithographed plates. This includes a handcoloured octavo folding, geological map, bound as a frontispiece, depicting the NSW coast down to Gippsland and Tasmania, a fold out single colour geological cross section of the Newcastle Coal Basin, 14 plates illustrating shellfish and flora and three tinted lithographs. Single page maps at the back illustrate wind patterns around Australia. Geoffrey Kaye was one of the contributors to this book, which included a foreword by F.H. McMechan, Secretary General, International Anesthesia Research Society. Blood pressure measuring enabled medicine to develop a greater understanding of shock and begin to develop more appropriate measures for resuscitation.
