We contacted UNSW Press, the book's publisher, which provided the following explanation. Apparently, the team there could not find the scan of the slide submitted for the book. However, a member of the team was able to capture a jpg of page 76 of the book. From my recollection of the original photo, the attached image is very close in terms of its reproduction quality.
