Duplicated SEC form titled "Application for Permission to Exchange Duties", form number S.115. Printed by a typed stencil or Gestetner process on duplicating paper. Headed SEC, Ballarat Tramways. Used for crews to formally seek approval for an exchange of shift or an exchange of days off with another crew, for a specific change of shift of with another shift.
