A brown coloured cardboard cover with black information on the front. Top right hand corner reads 7610.010-3675. Under the Australian Insignia is the information of the booklet. Bottom left hand corner there is a white label with 3550170 C414 AUS on it. There are two metal staples down the left hand side of the booklet and the booklet is covered with a plastic cover.
