A greyish coloured cardboard cover with black information on the front. Top right hand corner reads 7610-66-024-7173. Under the Australian Coat of Arms are the details of the booklet. There is a white label with 355.409 D6186:1 AUS typed on the label. The booklet has tow rusty metal staples down the left hand side and is covered with a plastic cover. Amendment No 1 is inside the booklet.
