A light blue coloured cardboard cover with blue information on the front. Top right hand corner reads DSN 7610-66-032-5158. Under the Australian Coat of Arms insignia is the description of the booklet. The booklet is held together with two metal screws on the inside. The booklet is covered with a plastic cover.
