A greyish coloured cardbord cover with black information on the font. Near the top right reads 7610-66-019-1535. Under the Australian Coat of Arms is the description of the booklet. There are two metal staples down the left hand side. The booklet is covered in a plastic cover and stored in a plastic bag.
