A bluish coloured cardboad cover with red information on the front. Near the top right hand corner reads WO Code No 8390. There is a whote label with 356.184 1431:F GRE tpyed on it. There are signs of water damage at the bottom of the booklet. There ae three punch holes and two metal staples down the left hand side. The booklet is covered with a plastic cover.
