A blue coloured cardboard cover with black information on the front of the cover. There is the Australian Army Insignia and under this are the details of the booklet. Top right hand corner reads DSN 7610-66-041-6416. There are two punch holes down the left hand side and the booklet is covered with a plastic cover.
