A mottled blue coloured cover with black information on the front. Top right hand side reads DSM 7610-66-032-5156. Under the Australian Military Forces insignia are the etails of the booklet. Bottom left hand side there is a white label with 356.110 G94614 AUS on it. There are two punch holes down the left hand side and the booklet is coved with a plastic civer.
