A blue coloured cardboard cover with faded edges. There iinformation on the cover is in black. Top right corner there is the number 1 which is circled this is done in black texta. Under this reads 7610-66-029-9975. there are two punch holes down the left hand side and the booklet is held together by two metal staples.
