A blue coloured cardboard cover with black informaion on the front. Top right hand corner reads DSN 7610-66-092-6271. Under the Australian Army insignia are the details of the booklet. Near the left hand bottom there is a white sticker with 355.504 D59817 AUS typed in black ink. There are three metal staples down the left hand side and the booklet is covered with a plastic cover.
