A faded blue coloured cardboard cover with black information on the front. Top right hand side reads DSN 7610-66-065-9677. Under the Australian Army insigia are the details of the booklet. There are several pages that have become loose from the booklet. there is what appears to be a water stain on the top left hand corner and there are two holes down the left hand side.
