A blue coloured cardboard cover which has faded around the edges. The information on the front of the cover is in black. Under the Australian Army Insignia are the details of the booklet. Top right hand corner reads DSN 7610-66-048-1189/ Under this is 6 TPT COLM which has been stamped om in blue ink. There are two punch holes down the left hand side of the book.
