A faded blue coloured cardboard cover with black information on the front. Handwritten in read text reads 37 A.D .Pl which is underlined in red. Top right hand side there is a stamp which reads 6 TPT COLM and under this is DSN 7610-66-034-5750. There are two punch holes and one metal staple down the left hand side of the booklet.
