Printed form on off white sheet (two copies joined at top - ex pad?) titled "The Met - Weekly Record of Employees Ticket Outfit". Has space for employee name, out fit numbers dates, week ending and all type of zone tickets, including getabouts, short trips, checks and MCG ticket. Form No. F/PTC060218
Second version - form 3/149A dated 9/1992 added, similar layout.
