Form - printed on manila A4 coloured card, folded in half to form an A5 sheet, titled "M>Tram Training Card on-the-job training record". Has space for the trainee, trainer, tram class, details and a tram driver check list of items to be checked. The inside part allowed for training details to be recorded.
