Two blue and one white pieces of paper glued together. Doctor's Certificate from H L Atkinson certifying that Hugh Peters was unable to follow his employment and dated March 4/66. Letter from Hugh Peters dated 4th March 66 claiming Sick Allowance, also signed by James Sampson. On the third piece Hugh Peters declares himself off the Sick fund of the Court. Dated March 13/66.
