A blue coloured cardboard cover with black information on the front. Top right hand corner reads 7610-66-024-7346. Under the Australian Coat of Arms are the information of the booklet. There are four thick black lines across the fromt of the cover which have been done with a black texta. The booklet is covered with a plastic cover. Some of the pages have come away from the booklet and are lose. The name Christopherson is written inside the cover.
