Form for reporting accidents for both trams and buses, dated August 1975, form No. 3/263. Provides spaces for information about a traffic accidents, witnesses, vehicles, diagram of the scene, details of the event, damage, and for a general description of the accident with space for signatures. Asks Depot Masters to see that form is fully and intelligently made out.
