A blue coloured cardboard cover with black information on the front. Top right hand corner reads DSN 7610-66-034-9289. Under the Australian Army Insignia are the details of the booklet. There are two seperate amendments at the back of the booklet. The booklet and amendments are held together with a metal slide.
