A blue coloured carboard cover with red information on the front. Top right hand corner reads 67/17/101 in black ink and under this reads 7610-66-010-2544. Top left hand corner there is a stamp which reads MON U Coy. The details of the booklet are under the restriction notice. There are two metal staples down the spine of the booklet. Amendment No: 1 is inside the booklet.
