The medal has a large voided 'R' in the centre with a crown on top, surrounded by the words Queen Alexandra's Imperial Military Nursing Service Reserve. The medal and ribbon sit inside the lower half of a Royal mint box on a blue cushion with the Royal Mint logo stamped on the upper inside and a red covering on the outside of the box.
