The certificate lists the members of the Supplejack Tent No.401 Dumbalk who have volunteered for "Active Service in response to the Empire's Call." Names handwritten in black ink. The border is decorated with coloured flowers and flags and the images of King George V, the Minister for Defence and the first Australian soldier to win a VC.

Denis Gibbons (1937 – 2011) Trained with the Australian Army, before travelling to Vietnam in January 1966, Denis stayed with the 1st Australian Task Force in Nui Dat working as a photographer. For almost five years Gibbons toured with nine Australian infantry battalions, posting compelling war images from within many combat zones before being flown out in late November 1970 after sustaining injuries. Put together by a member of the Eltham Gateway Action Group (EGAG) in participation with various artists, writers, local business people and other local citizens and sent to Minister Maclellan, Planning Minister in the Kennet Government in protest of proposed plans to sell the former Eltham Shire Office site at 895 Main Road Eltham in order to develop a Shell Service Station and Hungry Jacks fast food outlet. The purpose was to express what is unique about the character of Eltham, why people live and work there and why such a development would be out of character and inappropriate for the Eltham Gateway along with alternative development proposals. David Macknight Jones was born on 31 January 1910, the son of the Rev. David Samuel Jones. He attended University High School, then Ormond college, where, after graduating in Arts, he studied Theology. He obtained his Bachelor of Divinity in 1936, and was ordained in St. Stephen's Church Toowoomba, where his father was the minister. He married Elizabeth Ann McLellan in 1938, and in the same year was inducted as the first Minister in the Western patrol (Timboon), Presbytery of Mortlake, Victoria. He was called to Scots Church Shepparton in 1941, to St. Andrew's Launceston in 1947, to Kew Presbyterian Church in 1950 and to Pascoe Vale in 1965. Albert Jaime Grassby, AM was an Australian politician who served as Minister for Immigration in the Labor Whitlam Government. He completed reforms in immigration and human rights, and is often known as the father of Australian "multiculturalism".
