Certificate, Proficiency, Anaesthesia, 1906
At the time of issue, anaesthesia was a practice still in its infancy. Proficiency in anaesthesia was determined by successful demonstration of a small number of administrations. It was also built into the existing medical qualifications and not a specialty in its own right. Dr Montefiore David Silberberg completed his training at the Melbourne and Children's Hospitals and went to London where he was clinical assistant to Sir James Mackenzie and was associated with Sir Thomas Lewis who did ground-breaking work on digitalis and electrocardiography. When he returned to Melbourne, he brought with him the first electrocardiograph used in medical practice in Australia. He started working in Collins Street and at the Melbourne Hospital as a specialist in heart problems. During World War I, he served in Egypt in 14 Australian General Hospital 1914-16. In 1919 he was appointed physician to outpatients at the Alfred Hospital while also working at the Children's Hospital and at Repatriation General Hospital. He was appointed inpatient physician at the Alfred Hospital in 1925 and, at about the same time, a University of Melbourne Stewart Lecturer in Medicine. He mentored many young cardiologists during his lifetime, providing instruction at the hospitals where he worked and holding regular extra-curricula meetings at his home.Handwritten certificate from the University of Melbourne acknowledging the proficiency of Montefiore David Silberberg in the administration of anaesthesia. Insufflation anaesthesia machine
In 1913, Mark Cowley Lidwill designed a machine for the purpose of mechanical or insufflation anaesthesia. The Lidwill machine was a portable machine weighing 7kg that could easily be packed into two small bags. The machine involved compressed air being delivered to an ether vaporiser. An ether/air control device allowed varying concentrations of ether to be delivered. The ether vaporiser could be immersed in hot water to prevent cooling and the ether temperature was measured. Equipment - Ether in Oil
Ether in oil was used for rectal anaesthesia. Rectal anaesthesia offered a way to administer anaesthesia when using a mask was impractical, such as oral or respiratory tract surgery. Undiluted ether was irritating to the bowel and even proved to be fatal. Ether in oil, developed in 1913, minimized irritation with no reported deaths.Empty clear glass bottle with cork stopper which has become dislodged and is now inside the bottle. Doyen's mouth gag used by Dr Mitchell Henry O'Sullivan
A mouth gag was used when required to assist in the administration of anaesthesia to a patient. It is used to keep the patient's mouth open, and could also be used for oral surgery or airway management. This particular mouth gag is known as a Doyen's type.Dr Mitchell Henry O'Sullivan worked in the Victorian country town of Casterton as a general practitioner from 1919 until his death in 1977. He also practiced obstetrics. His son, Dr David More O'Sullivan donated the obstetric bag and its contents to the College in 1999. The gladstone bag and contents are a unique time capsule of the type of instruments and pharmaceuticals used in the inter-war period.Stainless steel mouth gag. The image, without a blackened out background was reproduced on page 21 in 'Practical Anaesthesia'. Practical Anaesthesia was the first text book on anaesthesia produced in Australia. Geoffrey Kaye was one of the contributors to this book, which included a foreword by F.H. McMechan, Secretary General, International Anesthesia Research Society. Blood pressure measuring enabled medicine to develop a greater understanding of shock and begin to develop more appropriate measures for resuscitation. Book - Case book: Joseph Clover, Medical Observations J.T. Clover, 1846
Dr Joseph Clover was an early pioneer of anaesthesia, particularly chloroform. Like most physicians, he recorded his work in a casebook. This casebook records Clover's first administration of ether as anaesthesia. The book was given to Professor Robert Mackintosh by Mary Clover, Joseph Clover's daughter. along with other casebooks. Mackintosh then proceeded to re-gift the casebooks and they are now dispersed. Clover wrote this casebook when he was a medical student, before he trained as a surgeon and around the time that anaesthesia was discovered. He eventually became the most important authority on anaesthesia in Britain and is featured on the crest of the Royal College of Anaesthetists. This artefact associated with one of the world's foremost pioneers of anaesthesia is historically and scientifically significant on an international level, and is significant for its rarity, condition and research potential. Hewitt’s airway was the first known oral airway and laid the foundations for the numerous other airways that were later developed. The development of the oropharyngeal airway is one of the lesser-acknowledged advances in anaesthesia. Oral airways made anaesthesia safer and significantly reduced the trauma associated with earlier attempts at managing the obstructed airway. The Hewitt Airway – the first known artificial oral ‘air-way’. In 1890, Frederic Hewitt noted the frequency and danger of upper airway obstruction. In 1908, dissatisfied with the use of tongue forceps and that Esmarch’s maneuver sometimes failed, he described the first oropharyngeal airway, the Hewitt Airway. The tube was less than 8 cm long, ideally connecting the oral opening to the hypopharynx. Francis Percival de Caux invented this endotracheal tube which was devised for use during nitrous oxide/oxygen anaesthesia. Though he wasn't the first to use a two-tube method of anaesthesia, de Caux's invention was particularly effective; in 1930 he reported having given 20,868 nitrous oxide/oxygen anaesthetics in a four year period without a fatality. A long flexible metal tube formed from tightly wound metal with a small bulbous introducer at the distal end. The tube is mostly encased in a metal and rubber sheath. In 1966 Harry Daly was admitted to the Order of St Michael and St George in recognition of over 35 years of service to medicine and anaesthesia.Large white enamelled seven armed Maltese Cross set around a dark blue ring bearing the Latin motto 'Auspicium Melioris Aevi'. The obverse depicting St Michael standing on the devil and the reverse St George on horseback killing a dragon. The medal comes with its blue-crimson-blue ribbon folded and stored under the velour badge holder. Medal and ribbon are set and stored in its custom made black case with the badge acronym in gold lettering on the top of the case [C.M.G.]. General anaesthesia relaxes muscles around the airway which can result in the airway becoming obstructed. Holding the jaw open or keeping the tongue out of the way were crucial. Often, brutal devices were used to open the jaw or pull the tongue forward to clear the airway.Set of four chrome plated metal tongue depressors of different sizes. Mask, Wire, Ether, 1910
Mask used for administering ether anaesthesia. This mask also has a carbon dioxide inlet tube. Carbon dioxide was found to promote deeper breather which assisted in attaining faster anaesthesia. The mask was designed by an American anaesthetist James Tayloe Gwathmey, (1863-1944). Gwathmey invented a range of anesthesia equipment. In 1904, he introduced this mask as one part of a resuscitation apparatus. Gwathmey modified an existing mask to more closely fit the contours of the face. The holes in the rim allow for oxygen to be delivered for resuscitation or for the delivery of a combination of oxygen and anaesthetic. Wire framed mask in tear shape. There is a hinged top bracket which allows for a piece of flannel or domette to secured to the mask, onto which the ether would be administered. Plaster cast of the head of the first baby to be delivered under anaesthesia by the use of ether, 1847
This a cast of the first baby to be delivered under anaesthesia, by the use of ether, in Edinburgh on 19 January 1847. The famous physician James Young Simpson, Professor of midwifery at Edinburgh University, attended this birth and wrote about it in the Monthly Journal of Medical Science 1846-7 Vol.7, p649-640. The cast of the baby's head was given to Lance Townsend, Professor of Obstetrics and Gynaecology, University of Melbourne by Robert Kellar, then Professor of Midwifery and Diseases of Women at the University of Edinburgh, when Professor Townsend was visiting Edinburgh. There is at least one other plaster copy; one is located at Wood Library-Museum of Anesthesiology, 520 North Northwest Highway, Park Ridge, IL 60068-2573, USAReplica of a new born baby's head, painted plaster, life size. The model of the head shows a large indentation of two and a half inches in the skull on the left side. Inhaler, Hewitt's (modified), Coxeter
Sir Frederick William Hewitt (1857-1916) wrote one of the earliest comprehensive textbooks on anaesthesia, and designed several pieces of anaesthetic equipment. In 1901, he described his wide-bore ether inhaler, a modification of the Clover Ether Inhaler. Unlike the Clover, it can be refilled with ether while still in use, and the mask is screwed into place so that it "cannot be unexpectedly detached." This version is a modified Hewitt's Inhaler which has a narrower 'ways' than the original Hewitt's, and no water compartment. Additionally, one side of the bowl is made of glass which enabled the person administering anaesthesia to see the level remaining in the bowl. Bowl shaped inhaler with a glass bowl base and metal dome top with a connector which appears to be for a rebreather bag. Experiments with intravenous anaesthesia were undertaken in 1872 using chloral hydrate. The high mortality rates discouraged further experimentation until the early 20th Century. Thionembutal, or Pentothal Sodium, was one of these early developments. This ampoule has "Experimental" stamped on it. It has come directly from the manufacturing process. Pentothal Sodium is its trade name.
