This is a foolscap size booklet. The front cover is white cardboard with the Royal Crest. The printing is in black ink and there is a decorative rectangular around the title. Inside are 12 pages. Printed in block letters are the Rank and Names for Honours. In lower case is a brief paragraph as to why the award was given.
