Physical description

Black leather folder. Logo of the Alfred Hospital stamped in centre and recipients name at bottom right of front [gilt ink] Inside there is a printed certificate, with handwritten date and signatures. Folder contained in a black faux leather envelope along with other certificates in a pocket in the inside bottom half of folder.
