Jack received his OAM on Australia day 1995 from the Honourable Richard McGarvie (refer Cat No 8045P) for his work as a volunteer with ex service personnel in the Bendigo RSL Sub Branch at the Soldiers Memorial Institute Bendigo.

Wooden frame, glass front, cardboard backing, White certificate to John Kerr Barnes printed in red, blue / gold emblem with crown on top,stamp top LHS. This is a copy of the original.
